PARTICIPANT AGREEMENT **Trip Overview** I understand that I am participating in a guided travel and humanitarian experience organized by *We Are Zion* involving activities such as hiking, boating, swimming, cultural immersion, service projects, and other group activities. **Participant Responsibilities** I agree to: * Follow all instructions of trip leaders and guides. Failure to comply may result in dismissal from the trip at my own expense with no refund. * Treat fellow participants, leaders, staff, local communities, and the environment with respect at all times. *Violence, verbal abuse, bullying, harassment of any kind, or mistreatment of leaders, participants, locals, tourists, animals, or others will not be tolerated and will result in early dismissal without refund and possible permanent expulsion from the program. ” * Disclose in writing any relevant medical conditions, allergies, medications, or physical limitations prior to the trip. * Comply with all laws and regulations of the host country and respect cultural norms. **Payments and Refunds** I acknowledge the payment schedule, cancellation policy, and refund structure as communicated to me in writing. I understand that all fees must be paid in full prior to departure. I further acknowledge that once the trip has commenced, no refunds will be issued under any circumstances. **Travel Insurance Requirement** I agree to secure and provide proof of personal travel insurance covering medical emergencies, evacuation, and trip interruption before departure. **Assumption of Risk** I understand that travel involves inherent risks including but not limited to: injury, illness, accidents, loss or damage of property, unpredictable weather, natural disasters, political unrest, and in rare cases, death. **Acknowledgment** By signing, I certify that I have read, understood, and voluntarily agreed to the terms of this participant agreement. — ### 2. LIABILITY WAIVER & INDEMNITY AGREEMENT **1. Assumption of Risk** I acknowledge and accept that participating in this trip involves risks, including but not limited to: accidents, illness, property damage, natural hazards, exposure to communicable diseases, and injury. **2. Waiver and Release** I voluntarily release and hold harmless *We Are Zion* , its directors, staff, volunteers, contractors, affiliates, and partners from any claims, damages, or liabilities that may arise from my participation, except in cases of gross negligence or willful misconduct. **3. Indemnification** I agree to indemnify and hold harmless *We Are Zion* from any claims brought against them as a result of my actions, conduct, or failure to disclose relevant information. **4. Binding Effect** This agreement is binding upon me, my heirs, executors, administrators, successors, and assigns. — ### 3. MEDICAL RELEASE & EMERGENCY CONTACT INFORMATION **Emergency Medical Authorization** * In the event of illness or injury, I authorize *We Are Zion* staff or designated leaders to obtain emergency medical treatment on my behalf. * I understand that I am responsible for any medical expenses incurred during the trip. **Disclosure of Medical Conditions** I certify that I have disclosed all relevant medical conditions, medications, and allergies. **Emergency Contact Details** Name of Emergency Contact: **Parent/Guardian Information** **Consent to Participate** I, the parent/guardian of the above-named minor, consent for them to participate in the *We Are Zion. I authorize emergency medical treatment for my child if necessary. I release *We Are Zion* and its staff from liability arising from such treatment, except in cases of gross negligence or willful misconduct. **Travel Responsibility** I acknowledge the inherent risks and agree not to hold *We Are Zion* liable for injury, accident, illness, or other issues, except in cases of gross negligence or willful misconduct. **Behavior Expectations** I understand that my child must follow the rules set by the organization. If their behavior is deemed unsafe, inappropriate, or harmful, they may be sent home at my expense without refund. — ### 5. JURISDICTION & GOVERNING LAW I agree that this agreement and any disputes arising from participation in *We Are Zion* programs shall be governed by and construed under the laws of the state of [Insert State/Province], without regard to conflict of law principles. I further agree that any claims or legal actions shall be filed exclusively in the courts of [Insert County/State/Province], and I submit to the jurisdiction of those courts. — ### 6. CODE OF CONDUCT & BEHAVIOR POLICY I agree to abide by the following rules: * No use or possession of illegal drugs, alcohol, or tobacco products. * No weapons, explosives, or dangerous items. * Respect for curfews, schedules, and instructions from leaders. * Respect for dress codes and cultural norms of the destination. * No harassment, bullying, or inappropriate relationships. * Compliance with local laws and customs. Violation of these rules may result in immediate dismissal from the program at my expense, without refund. — ### 7. MEDIA RELEASE AGREEMENT I grant *We Are Zion* permission to photograph, video record, and otherwise capture my likeness during program activities. I consent to the use of these images and recordings for educational, promotional, or marketing purposes in print, digital, and social media formats. I waive any right to royalties or compensation for such use. — ### 8. PROPERTY & PERSONAL BELONGINGS DISCLAIMER I acknowledge that I am responsible for my personal belongings at all times. *We Are Zion* is not responsible for loss, theft, or damage to personal items, including luggage, electronics, or valuables. — ### 9. TRANSPORTATION & TRAVEL DISCLAIMER I acknowledge that travel involves risks associated with flights, buses, vans, boats, and other forms of transportation. *We Are Zion* is not responsible for delays, cancellations, missed connections, or accidents outside of its control. — ### 10. EMERGENCY PROCEDURES & EVACUATION CLAUSE I acknowledge that leaders may, at their discretion, alter the itinerary, reroute, or evacuate participants due to weather, political unrest, natural disasters, or safety concerns. I agree to assume financial responsibility for any additional costs incurred as a result of emergency changes. — ### 11. HEALTH & FITNESS DECLARATION I affirm that I am physically and mentally capable of participating in program activities. If unsure, I have consulted a doctor prior to travel. I accept responsibility for my own health and safety during the program. — ### 12. COMMUNICABLE DISEASE ACKNOWLEDGMENT I understand that participation may expose me to communicable diseases, including but not limited to COVID-19. I agree to follow all safety protocols required by *We Are Zion* , local authorities, and host organizations. — ### 13. FINANCIAL RESPONSIBILITY STATEMENT I agree to pay all program fees in full before departure. I am responsible for any additional expenses incurred during the trip, including but not limited to: medical treatment, emergency evacuation, or early return home due to misconduct or personal choice. — ### 14. SPECIAL DIETARY & ALLERGY RESPONSIBILITY I acknowledge that *We Are Zion* will make reasonable efforts to accommodate dietary restrictions and allergies if disclosed in writing before the trip. However, I understand that full accommodation may not be possible and I am ultimately responsible for managing my dietary needs. — ### 15. SEVERABILITY CLAUSE If any provision of this agreement is found unenforceable or invalid, the remainder of the agreement shall remain in full force and effect. — ### 16. ACKNOWLEDGMENT OF VOLUNTARY PARTICIPATION I acknowledge that my participation is voluntary. I may withdraw prior to departure subject to refund policies, but once the trip has commenced, I understand that no refunds will be issued. — ### 17. DISPUTE RESOLUTION CLAUSE Any disputes arising from this agreement shall first be resolved through good faith negotiation. If unresolved, disputes shall be submitted to mediation or binding arbitration before legal action is pursued. — **Final Acknowledgment** By signing below, I confirm that I have carefully read, understood, and voluntarily agreed to all terms outlined in this packet.
